Despite the large amount of studies and a worldwide research that lasts since the 60s, a sensitive and specific marker for hepatocellular carcinoma to be used in the population at risk (the cirrhotic patients) is still lacking. Most of the available ones have shown a prognostic role, indeed, and a number of new molecular markers have recently been proposed, again more adequate in giving a prognostic definition than in being diagnostic tools. These new molecular markers, however, may represent the starting point for the targeted therapy of HCC and their clinical impact is probably going to increase.
