This presentation addresses the challenges of diagnosing lupus anticoagulant (LA), a key laboratory criterion for antiphospholipid syndrome (APS). It covers the complete testing process from pre-analytical considerations to analytical methods and post-analytical interpretation. Special attention is given to the growing problem of direct oral anticoagulant (DOAC) interference, which can cause false positive/negative results, and the effectiveness of DOAC removal methods. The presentation emphasizes appropriate testing strategies including screen-confirm-mix protocols, reference interval establishment, to improve detection of LA and minimize both false positives and negatives.
